ডাঃ শামীম বক্স – এর ওয়েভ পেজের ফর্মে উল্লেখিত স্থানে রোগীর নাম, ফোন নাম্বার, বয়স, ঠিকানা পূরণ করে বাটনে ক্লীক করিতে হইবে। ওয়েভ পেজে তথ্য-পূরণ ফর্মে সিরিয়াল তালিকাভূক্ত করার সময় মোবাইলে ফোনে SMS এর মাধ্যমে প্রাপ্ত ভেরিফিকেশন কোড নাম্বারটি বক্সে অবশ্যই লেখিতে হইবে এবং এরপর ক্লীক করে সিরিয়ালের ভেরিফিকেশন নিশ্চিত করিতে হইবে। অন্যথায় ঐ তালিকাভুক্তি বাতিল বলিয়া গণ্য হইবে। ডাঃ শামীম বক্স – এর ওয়েভ পেজ থেকে নির্দিষ্ট দিনের জন্য একটি মোবাইল নাম্বারের মাধ্যমে সিরিয়াল তালিকাভুক্ত করিতে হইবে। এরপর সাক্ষাতপ্রাপ্ত তারিখ-সময়ের একঘন্টা পূর্বে ”বক্স ফার্মেসী” থেকে রেজিস্ট্রেশন পেপার অবশ্যই সংগ্রহ করে ডাক্তারের চেম্বারে নির্দিষ্টে সময়ে উপস্থিত থাকিতে হইবে এবং সাক্ষাতকালে সংগৃহীত রেজিস্ট্রেশনের কাগজটি চেম্বারে প্রদর্শন করিতে হইবে। তালিকাভুক্ত হয়েও কোনো রোগী যদি নির্দিষ্ট তারিখে রেজিস্ট্রেশন পেপার সংগ্রহ না করে, তবে ঐ রোগীর নাম, ফোন নাম্বার এবং ঠিকানা ওয়েভ পেজে ব্লক করে রাখা হইবে। এতে ঐ রোগী পুনরায় ডাক্তারের সিরিয়ালের জন্য তালিকাভুক্ত হইতে আর সুযোগ পাইবে না।

In the form on Dr. Shamim Box’s webpage, the patient’s name, phone number, age, and address must be filled in, and then the button must be clicked. When filling out the information form on the webpage for serial listing, the verification code received via SMS on the mobile phone must be entered in the box, and then the serial verification must be confirmed by clicking. Otherwise, the listing will be considered canceled.

To register for a serial through Dr. Shamim Box’s webpage, you must list a mobile number for a specific day. Then, one hour before the scheduled appointment time, the registration paper must be collected from “Box Pharmacy” and the patient must be present at the doctor’s chamber at the specified time. The registration paper collected must be shown at the chamber during the appointment.

If any patient who is listed fails to collect the registration paper on the specified date, their name, phone number, and address will be blocked on the webpage. This will prevent the patient from being able to register again for the doctor’s visit.
